Summary:
Responsible for providing care coordination, bonding and advocacy services as aligned with the agency mission, goals and purposes.
The items listed below provide an overall summary but not comprehensive list of the required tasks.
Care Coordination Care coordination is a core responsibility of a change engineer.
Every engineer is expected to complete all duties in accordance with COI protocols and best/proven practices.
Facilitate and coordinate the care of COI families, focusing intently on the needs of the children.
Assist clients in resolving issues by identifying and accessing resources that meet needs, reduce risk, and promote success of participants.
Effectively engage children, caregivers, incarcerated caregivers, and other household members in offered services.
Using COI talking points and professional knowledge, explain (to children, caregivers, incarcerated parents and other interested parties) the program and activity purpose and value.
Engage community stakeholders to enhance services to existing clients and expand the number of families served.
Set and achieve targets to expand client base so that more families in need are served.
Interface with other service providers, agency networks, business community and other resources providers to facilitate successful progress of participants.
Conduct outreach to identify new clients; explain and discuss services.
Support development and maintenance of accurate, thorough, and up-to-date community resource databases and information resources.
Ensure completion of all required participant reporting forms within established time frames.
Develop case plan incorporate data gathered from assessments and specific client input.
Write, edit, manage and up-date case notes in COI case management system.
Demonstrate teamwork and professional development Participate in COI care coordination activities.
Prepare for and participate in COI case coordination and review meetings.
Participate in COI ongoing learning opportunities to enhance service delivery.
Complete other care coordination tasks, as assigned Family and Caregiver Bonding Family and Caregiver Bonding is another core responsibility of a change engineer.
Every engineer is expected to complete all duties, including support tasks as requested, with the highest professional demeanor, to the best of their abilities, and in accordance with COI protocols and best/proven practices.
Actively engage with clients to provide supportive services they require and model professional behavior Actively and positively participate, engage, and serve as role model during the programming efforts at the COI bonding events.
Attentively observe, consider, and note interactions, affect, and actions of program participants for consideration.
Work closely with and help participants to complete surveys, observational forms, assessments and other documentation regarding each incarcerated parent/caregiver participant, child/parent/caregiver interaction, etc.
, as requested prior to, during, and following the bonding program.
Assist clients in resolving issues based on needs assessment, including identifying and accessing resources that meet needs and reduce risk of participants.
Work with incarcerated parents/caregivers to develop Individualized case plans based on asset assessment for successful re-entry.
ICP should include specific achievable benchmarks and outcomes as well as feasible timelines.
Proficiently complete all bonding support responsibilities Assists with the logistical planning, preparation and coordination of the child-focused bonding visits as requested.
Contact children and caregivers to coordinate child-focused bonding visits, as scheduled.
Continuously develop professional knowledge, skills, abilities, and network.
Participate and fully engage in pre-activity training efforts on programming.
Interface with other service providers to facilitate successful bonding activities Educate and Advocate Every engineer is expected to actively seek out opportunities to educate the public and advocate for children of incarcerated parents by raising awareness of their challenges and successes.
Participate in efforts to raise the profile of Children of Inmates, Inc.
such as advocacy events, Obtain client testimonials and photographs.
Participate in conferences, meetings, and other external activities.
Prepare or help prepare presentations, articles, videos, and other media outlets General Duties Participate in COI teams and complete team assignments, as requested.
Report any suspected child abuse or neglect, as required by law.
Maintain established operational systems and support efforts to improve or create new systems to effectively support COI processes.
Complete or facilitate completion of all required participant reporting forms within established time frames.
Participate and complete tasks assigned as part of COI program development teams to support the evolution of the COI program model.
Serve as a role model, task manager, and mentor to COI interns and volunteers, as requested.
Complete other tasks as assigned Requirements:
Minimum of a Bachelor Degree and/or 2 years of case management experience or relevant community/field experience.
Willingness to travel, (must have reliable transportation).
High level of interpersonal skills to handle sensitive and confidential information.
Strong analytical and problem solving skills.
Excellent written and oral communication skills.
Ability to interact and communicate with individuals at all levels of organization and partner organizations.
Ability to foster diversity and open communication within teams and programs.
Ability to work individually and as part of a high performing team.
